Vitalink responds with the same version that is already present in the EPD. The local medication scheme and Vitalink medication scheme are in sync. | 2 | There is no local medication info. Vitalink holds a medication scheme Vx. | 2-3 | Patient is admitted to the hospital, Vitalink medication scheme Vx is downloaded. | 3-4 | Local EMR Home medication is possibly updated to Vx'. | 4-5 | During admission, Admission medication (= medication during an admission in the hospital) A is determined. This admission medication scheme will hold relevant information from the home medication Vx', possibly substituted to the clinical formularium. | 5 | During admission changes A to Z can happen to the admission medication.
